Here is an example of a specific documentation gap we’ve identified related to respiratory failure which is classified more specifically In ICD-10-CM than in ICD-9-CM. The respiratory failure codes in ICD-10-CM have been expanded to include codes for acute, chronic, acute on chronic, and unspecified types of respiratory failure at the 4th digit level. And they’re also further subdivided as to whether or not the respiratory failure is associated with hypoxia, with hypercapnea, or unspecified whether with hypoxia or hypercapnea.
